New Applicants: fill out Sections A and B ONLY on the Manifest/Change form when you have taken possession of your vanpool vehicle and/or are in the process of applying. You will gather your odometer readings for one day, commuting to and from work. After entering your information, save the form and email to SolVan@SunLine.org.
Vanpool Changes after SolVan approval: Once your vanpool is operating and on the road, should you have changes from your initial application, fill out the appropriate sections on the Manifest/Change form email to SolVan. Include ANY type of change to your vanpool, including passengers leaving the vanpool or new passengers joining the vanpool, a change in the Leaseholder, changes to your pickup and drop off locations, schedule changes and/or changes to your or your Employer's contact information. It is critical this form be updated and sent to SolVan and indicate the exact date these changes are to occur.
